Q: Does a 2-week “Under Review” turnaround time usually mean rejection?

Detailed Question -

I've recently submitted a manuscript to a reputable journal. The status went to "Under Review" in about 3 days after submission. By 14 days, the status had now been changed to "Decision in Process". This makes me nervous as it may indicate the editors decided that the manuscript did not meet their standards. Has anyone experienced this? Does that mean a rejection is usually the result in this scenario?
